Survey forms for nursing homes updated
The Centers for Medicare & Medicaid Services (CMS) has made updates to the Long-Term Care Survey Resources file. The updates include changes to the following forms:
- Staff Formula for F888
- Staff Matrix for F888
- LTCSP Complaints Guide
- Entrance Conference Worksheet
- FIC Folder (Staff Formula for F888, Staff Matrix for F888)
The COVID-19 staff vaccination matrix instructions now clarify to not include contracted staff. Temporary delay of vaccination is further defined as a delay due to clinical considerations including known COVID-19 infection until recovery from the actual illness and criteria to discontinue isolation have been met. An additional clarification included the explanation that facility staff who have been suspended or are on extended leave e.g., Family and Medical Leave Act (FMLA) leave, or Worker’s Compensation Leave, would not count as unvaccinated staff for determining compliance with this requirement. A new complaint guide document has been added to the surveyor resources folder that includes instructions for transferring complaint and FRI information from the ASPEN Complaints/Incidents Tracking System (ACTS) into the survey so that the survey team has the information while on survey. The entrance conference worksheet added a new item now labeled as item number 36 which requests a list of contract companies that provide services to the facility/residents. Identify the name of the contract company; whether the company provides direct care or non-direct care; how often services are provided (e.g., daily, weekly); the approximate number of contract staff provided by the company; and information on how the facility ensures contractor staff are compliant with the vaccination requirement.
